Many lifelong needle-phobes end up barely noticing their weekly injection after a few months. The anticipation is almost always worse than the reality. The needles are tiny, and the whole process takes about 30 seconds.
That said, proper technique matters. Here's what you need to know.
Where to inject.
GLP-1 medications go subcutaneously, under the skin, into fatty tissue. The approved sites:
Abdomen (most popular): at least 2 inches from your belly button in all directions. Large area for rotation, consistent absorption, easy to reach.
Front/side of thighs: middle third, outer surface. Good alternative when your abdomen needs a break.
Back of upper arms: harder to reach alone but works well for rotation.
Why rotation matters.
Using the same spot repeatedly causes lipohypertrophy: hardened lumps of fatty tissue that reduce medication absorption. This means unpredictable blood levels and potentially less effectiveness.
Simple rotation system:

Step by step.
Before:
- Wash hands with soap and water
- Check medication: not expired, clear and colorless, no particles
- Let it reach room temperature (15-30 minutes out of fridge, reduces stinging)
During:
- Clean site with alcohol swab, let it dry completely (wet alcohol stings)
- Pinch skin to create a fold
- Insert needle at 90° angle (45° if you're very lean)
- Press the injection button fully
- Hold for 10 seconds after the dose counter hits zero. Removing too early causes medication to leak.
- Remove smoothly. Don't rub the site.
After:
- Light pressure with cotton if there's bleeding (normal)
- Dispose needle in sharps container immediately
- Store pen properly (most can stay at room temperature after first use; check your specific medication)
Common mistakes.
Removing the needle too quickly. The most common mistake. Count to 10 after the dose delivers. Impatience here means lost medication.
Same spot every time. Even rotating between just 2-3 spots isn't enough. Use the full rotation schedule.
Injecting through clothing. Always expose and clean the skin.
Rubbing the site. Causes bruising and can affect absorption. Leave it alone.
About the needle fear.
If needles make you anxious, know that:
- GLP-1 pen needles are very thin (29-32 gauge) and short (4-8 mm)
- Most people report minimal to no pain
- The auto-injection pens do most of the work. You press a button, not push a plunger
- The fear typically fades by injection 3 or 4
